Dear Members,

As a leader within Pediatric Oncology at your site, we would be grateful if you would take the following brief survey on dexrazoxane use, along with forwarding to colleagues within your division.

We are conducting a voluntary, anonymous research survey to better understand current practice patterns around use of dexrazoxane in children and young adults with cancer.  We are surveying oncology physicians, fellows, and advanced practitioners to understand attitudes and barriers towards dexrazoxane use and to describe practice variability.  Your survey responses will not be linked in any way to your name or email, and data from this study will be presented as aggregate data in any presentations or publications.  Your participation will not impact your employment. You will not receive payment for survey completion.

Completion of the survey indicates your consent to participate in the study. The survey should take around 5 minutes.
